JOB SUMMARY
This position will work 18 hours per week on Tuesday, Wednesday and Thursday. The primary objective of the Behavioral Health Assistant is to provide administrative support for the Behavioral Health Team and the integrated mental and physical health care model. This role serves as a liaison for the behavioral health program with the Doctors Care Clinic and the Doctors Care Sliding-Fee-Scale Program.

JOB RESPONSIBILITIES
•	Maintain familiarity with Patient Assistance Programs and community resources, and share those with staff, volunteers and patients as needed
•	File Prescription Assistance Paperwork on behalf of patients, reorder medications as needed and ensure patients fill out renewal paperwork for these programs in a timely manner
•	Label medications, maintain a medication filing system and complete appropriate paperwork
•	Transmit and obtain medical records to and from other community providers 
•	Contact patients who have violated a team policy regarding their offense and educate on how to improve their behavior (i.e. missing appointments, cancelling late, etc.)
•	Obtain and communicate patient lab results 
•	Check Medicaid, grant or Sliding-Fee-Scale eligibility on scheduled patients prior to their appointment time
•	Work in coordination with the receptionist to schedule appointments and fill cancelled appointment slots
•	Maintain a “Cancellation Call List” of patients who would like to be notified if an earlier appointment time becomes available
•	Assist the Program Director in gathering data and statistics for grant reporting as requested
•	Enter in data and/or scan in forms for new patients into their electronic patient record
•	Maintain telephone and face-to-face meeting records in eClinicalWorks
•	Utilize volunteers to assist with job duties whenever possible and appropriate
•	Complete any other duties pertinent to the position as assigned by supervisor
•	Maintain confidentiality of proprietary information including HIPAA and patient information
Place reminder phone calls to patients for appointments, paperwork and other items
•	Follow up with patients who no-show or cancel their appointments to find out why, and keep data on the answers
•	Make photocopies and send faxes for labs, medical records and Prescription Assistance Programs

Position’s primary function/objective:
To give Social Work leadership in the residential program in general and in the designated peer group team specifically. To provide clinical and child-care management direction and supervision to the assigned counseling team and the therapy group. To provide family therapy services to assigned residents and their families.To provide family therapy services to assigned clients that have sexually abusive adolescents and their families to stop abusive behavior
 
Job responsibilities include:
Direct the orientation process with the new resident to include contacting parents and collateral parties, and coordinate with the Day Counselor.
Prepare case Service Plans.
Communicate Case service Plan goals/objectives to the group counselors.
Monitor case service plans on a regular, daily basis as to effectiveness, need for changes and for appropriate objectives.
Direct participation and provision of leadership with group therapy sessions and Positive Peer culture session.
Be a bridge of communication between the Consultant Staff (Psychologists and Psychiatrists) and the Counseling Team.
Group Therapy: facilitate and supervise offense specific group sessions, insure offense specific curriculum is utilized and documented.
Family Therapy Services: insuring that clients received 1-3 hours of family therapy services per week, work flexible hours to accommodate family therapy needs.
Court Hearings: attend court hearing, foster care reviews and related meetings.
Other duties as assigned.</description>
